Carlos Ceballos, 71, of North Haven, passed away peacefully on Friday, January 14, 2022 after living with Alzheimer’s disease for several years. He was the beloved husband of Nancy Ceballos for 35 years. Carlos was born in San Juan, Puerto Rico on October 17, 1950 and was the son of the late Elba Santini and Osvaldo Ceballos Sr. Devoted and much-loved father of the late Katrina Ceballos, Carissa Vega (Mauriel), Christopher Ceballos, and Jillian Ceballos (fiancé Yader). Doting grandfather (aka “Grampy Scampy”) of Soleil and Kai Vega. Loving brother of Osvaldo Ceballos (Terry) and Lynette Hoyt (Fred). Also survived by nephews Antonio Hernandez (Yesenia), Richard Ceballos (Melissa), niece Elizabeth Ceballos (Bruce Moore), and sisters-in-law Jeanne Clement and Carol Delfino, and his former wife Sonia Caban. Predeceased by his mother-in-law Anita Clement. Carlos obtained a B.A. from Fordham University, an A.A.S. from Long Island University’s Physician Assistant Program, and an M.P.H. from Yale University School of Epidemiology and Public Health. Carlos was a medical officer in the Connecticut Army National Guard and later worked as a Neonatal Physician Assistant at St. Francis Hospital in Hartford. He transitioned into public health administration, working at Fair Haven Community Health and the Connecticut State Department of Health Services. In 1992, Carlos became the Coordinator of School-Based Health Centers for the New Haven Public School system. During his tenure, Carlos expanded the program from two to 15 centers, providing equitable access to medical and dental care to students in New Haven. He retired from this position in 2014, having made a profound impact on the provision of health services for underserved families in the community. Carlos enjoyed photography, history, the outdoors, and charting the course on family vacations that touched on all three. He was a lifelong learner who enjoyed sharing his discoveries with his loved ones. Carlos was an active member of the Boy Scouts of America since he was a young boy and became a troop leader later in life. He was an active parishioner of St. John’s Episcopal Church, where he served on the Vestry, led a variety of community outreach projects, and participated in the Brotherhood of St. Andrew.
Due to COVID concerns, funeral services will be private at this time. The family is planning an outdoor Celebration of Life event in the summer for extended family and friends. We are so sorry for his family and friends loss. We met Carlos at Cub scouts and stayed friends since then. He was a very kind man always willing to listen to what you had to say with a big smile. He was also very caring and loving not only to his family but to everyone. We miss him. 😇🤗❤️🙏
Although I never met Carlos, I feel like I knew him through the wonderful memories shared through his loving wife. Working alongside Nancy, I can’t help but share my condolences to her and her family. What a wonderful person he was and what a tribute to his life you all are.
I had the pleasure of working with Carlos during his tenure at the New Haven Public School System as a partner for Yale New Haven Hospital affiliated sites. Carlos always had the best interest of those underserved students as he advocated and collaborated regarded School Based Health Centers. My heart goes out to his family and friends.

I am so sorry for your loss. I enjoyed working with Carlos for many years in School Based Health in New Haven. He was a steadfast advocate for providing excellent health to all. I appreciated his ability to focus on what was important, and also maintain a sense of humor, even in difficult times.
